Oh well? Nobody's name is attached to this photo, and the original post was clearly just intended to get a rise out of people. Nobody's getting doxed over this photo, and every comment here and in the OP is positive.
What's the worst that can happen? Oh no, someone on Twitter made fun of my appearance! No different than appearing in the background of the news, weather report cam, police cam, or CCTV.
Take away the right to take photos of random people in public, you lose so, so many useful rights that we absolutely need.
The fact that the worst thing you can imagine is a few mean words online is a failure of your own imagination, it's not because that's actually the worst possibility.
For example, your boss could be shown the picture and fire you, as happened to Mitch Stein in 2012. Crimes such as arson or assault are on the table as well, and would be considered hate crimes in such an event if drag were a protected class.

Considering that this picture has ended up on social media with people judging their appearance, that makes it an issue.
